Come and learn all about Advance Care Planning over morning tea with Nurse Practitioner, Kelly Arthurs from ANDCare.
The event will take place on Wednesday 12 November from 10am - 12pm at the Boronia Park Sports and Community Facility, Park Rd and Ryde Rd, Hunters Hill.
Advance care planning involves planning for future health care. The event will provide attendees with the opportunity to make decisions about the health care they would or would not like to receive if they were to become seriously ill and unable to communicate their preferences or make treatment decisions.
All attendees are also eligible for a follow up personal consultation appointment with the Nurse Practitioner on either Mon 17 or Wed 19 November.
Registrations are free and can be made at events.humanitix.com/acp-huntershill. This workshop is open to residents of the Hunters Hill and Northern Sydney community.
