      • For Private Vehicles: current NSW vehicle registration papers showing the name and current address of the applicant.
      • For Company Vehicles: current vehicle registration papers and a Company letter stating that you have sole use of the vehicle on the company letterhead with the ABN.

        Provide one of the following:
        • Current Driver's Licence
        • Residential Lease (within last 12 months)
        • Rates Notice (within last 6 months)
        • Utility Bill

    • Privacy: The personal information supplied on this form is required in order to provide the service requested. it will be used by Council staff and stored in our record system for only as long as necessary. During this time it can be accessed and amended by you. We will only give your information to a third party with your consent or if we are required to do so by legislation.
